前往 FinCEN 的提交受益人所有权信息报告 (File the Beneficial Ownership Information Report) 网站, 选择 File Online BOIR 在线提交.
Filing Information
选择 Initial report 的初次提交并下一步.
Reporting Company
选择 Request to receive FinCEN ID (申请获取 FinCEN ID).
填入 Reporting Company legal name (申报公司法定名称).
选择 Tax Identification type (税务识别类型), 并填入 Tax Identification number (纳税识别号).
建议选 EIN, 若还未申请到 EIN 可选 SSN/ITIN, 都没有则选 Foreign.
选择企业创建所在州以及填写企业地址.
Company Applicant(s)
01/01/2024 之前创建的企业记得勾选 Existing reporting company.
填入 Legal name and date of birth.
注意 First name 与 Last name 顺序.
选择居住地址并填入详细地址, 在国内的话选中国. 地址的缩写可参考 USPS.
选择外国护照并填入护照号码, 选择证件的签发国家并提供证件图.
Beneficial Owner(s)
填入 Legal name and date of birth & Residential address & Form of identification and issuing jurisdiction.
此处信息与上一步类似.
Submit
最终, 填入邮件地址以及姓名后提交即可.
记得保存结果页的最终文件作为备份.
